Requirements
Required Credential(s): Licensure/Certification/Registration:
Physician MD/DO with Prescriptive Authority credentialed from the Oklahoma Board of Medical Licensure and Supervision.
Preferred Credential(s):
Advanced Life Support.

Doctor of Medicine (MD) or Doctor of Osteopathy (DO) required.
